To determine the sensitivity and specificity of standard imaging studies in identifying major visceral and solid organ injury in blunt abdominal trauma (BAT), we reviewed the records of all patients treated for BAT at a level 1 trauma center from March 1, 1990, to March 1, 1993. During this period, 482 patients were treated for BAT. All patients underwent a plain abdominal radiograph and computed tomography (CT) of the abdomen. During this time, a total of 109 patients were examined with angiography (testicular or splenic). The results of the clinical examination and all imaging studies were compared with eventual operative and pathological findings. If the patient had a normal clinical examination, the following imaging studies were done: plain radiograph in 46 patients, CT in 252 patients, angiography in 92 patients, and US in 67 patients. Those with abnormal findings on any imaging study underwent exploratory laparotomy. Overall, there were 33 injuries (6.8%) identified in the 84 evaluable patients. The sensitivity of the initial clinical examination was 82%; the sensitivity of the plain abdominal radiograph was 25%; and the sensitivity of the CT scan was 54%. In the nine patients with abnormalities on the CT scan, angiography was performed. Its specificity was 77%. US was the most sensitive and accurate imaging study. The CT scan had a higher sensitivity in identifying injury to the retroperitoneum, whereas angiography was the most sensitive study for detecting injury to the pancreas and small bowel. The incidence of major visceral injury in BAT is 6.8%. Plain radiographs, CT, and angiography have low sensitivity for the detection of major visceral injury and are generally regarded to have poor positive predictive value.
